The salary range for this position is $155,000-$160,000 per year based on experience.   

Responsibilities

The Care Coordination Director is responsible for the overall holistic approach to care for residents and coordination of services from healthcare providers and clinical partners.

 

Coordinate Resident Care

  • Perform assessments and reassessments to determine resident needs and establish appropriate individualized service plans.
  • Responsible for coordination of care for external partners to ensure a seamless care experience for residents.
  • Advocate for residents’ holistic health, provide healthcare guidance, and facilitate virtual care visits as needed.
  • Act as a contact for issues related to resident care within the community.
  • Communicate to residents, families, department heads, resident services staff, and General Manager in an effective and respectful manner as needed.
  • Provide Wellness Coaching (upon completion of Mayo Clinic Wellness Coach Training) to residents as part of overall individualized service plan.

Lead Resident Services Department

  • Lead community infection control program to ensure adherence to local, state, federal regulations, and safety codes, as well as all appropriate policies and procedures.
  • Complete daily Quality Enhancement reviews to ascertain if the department is meeting state regulations and Atria policies.
  • Ensure adherence to company policy and State Health and Safety Codes, and federal regulations for assisted living communities.Review, approve, and conduct necessary follow-up on all incident reports.

Support the Community

  • Support hospitality, leasing, and sales processes with promotion of a positive image of Atria. Develop and maintain active community relationships to support sales teams, and participate in Sales Calls.
  • (Clinical Related Matters) – Partner with the Memory Care Director, Wellness Director, and Wellness Manager in maintaining active community and professional ties with clinical and non-clinical contacts.
  • (Clinical Related Matters) – Partner with the Memory Care Director, Wellness Director, and Wellness Manager in acting as a contact for issues related to resident care within the community.
  • Participate in Resident Needs Reviews.
  • Deliver and attend required training for self and employee partners.
  • Maintain positive relations with residents’ families and clinical partners.
  • Maintain active community and professional ties.
  • Remain educated and up to date on advancements in medical technology and apply it in daily practice
  • May drive company vehicle from community to social and other various destinations (only if required by community).
  • May perform other duties as assigned.

Qualifications

  • A registered nurse licensed to practice as required by state regulations.
  • Experience in direct patient care and assessments/observation of clinical conditions.
  • Experience at a department head level in Assisted Living or a related industry.
  • CPR certified and knowledge of basic first aid.
  • Experience coordinating educational events.
  • Must successfully complete CDC infection prevention training, Mayo Wellness Coaching training, and all Atria specified training programs.
  • Must complete all state required training to maintain licensure or certification.
  • Able to work flexible work hours due to demands of position.
  • Position requires driving responsibilities (may use Company provided vehicle and/or personal vehicle).
  • Must possess valid driver’s license.
  • Must satisfactorily meet and be in compliance with Atria motor vehicle policy standards.
